We are currently seeking a passional and caring Service Coordinator to work in our Group Day Support Program! Come join our growing community of care today! This position is great for a current DSP looking to grow or for a current case manager or supervisor looking to join a team where they can truly make a difference. Essential Responsibilities and Job Duties:Coordinate the development and implementation of service plans, objectives and methods necessary for providing effective services to clients.Acts as liaison with providers for process reporting and service planning.Assists in admission process and discharge planning.Participates in provider meetings, community meetings, and program activities essential for the operation of an effective program.Assist in the development the daily living program for each client.Completes quarterlies for each client served.
