Overview:

Provides age specific screenings, interventions and discharge planning to patients/families. Provides education, supportive and bereavement resources while assisting patients and family members adjustment to their illness including collaborating with other multidisciplinary team members in the provision of case management services.

Responsibilities:
  • Demonstrates the ability to collaborate with the multidisciplinary health care team in the development of an individualized plan of care for discharging patients.

  • Provides advocacy by ensuring the patient and family are actively involved or represented in planning for care.

  • Responsible for identification and providing information to the patient/family to assist in resolving financial issues as they may relate to the inpatient outpatient care and discharge planning outcomes.

  • Participates in the self directed team work approach to assure Care Coordination

  • Plan for appropriate post discharge supportive services.

  • Document patient/family by contact, intervention, plan, and post discharge arrangement in the patient's medical record.

  • Coordinates Care Conferences for patients/families to ensure addressing discharge needs and facilitation of plan of care, within scope of SW licensure.

  • Assists with pursuit of guardianship, when appropriate, for the welfare of the patient. Assists patient in the development of Advance Directive and assists staff in identifying and appointing a Surrogate Decision Maker.

  • Serves as a liaison for causes of abuse or neglect in all patient populations within scope of SW licensure.

  • Coordinates functions regarding adoption within the scope of the SW licensure.

  • Proactively identifies and participates in process improvement to manage positive patient outcomes within the scope of SW licensure.

  • Assists hospital staff with identification of Surrogate Decision Makers.

Education and Experience:
  • Bachelor’s degree in Social Work from an accredited college or university.
  • Two years related experience and/or training in acute care setting.

Certification/Licensure:

Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) as granted by the Illinois Department of Professional Regulation required.
