Certified Procedure Coder (CPC) Certification required or

RHIA or RHIT required

If RHIA or RHIT then CCS is required wiithin one year from date of hire.

incumbent must have RHIA or RHIT, or related certifications.

Work Environment


Primary Responsibilities

Review medical record clinical documentation and diagnostic results as appropriate to extract data

and apply appropriate ICD-9-CM and CPT codes for billing, internal and external reporting, research,

and regulatory compliance. Under the direction of the Director of Coding and Data Operations,

accurately code conditions and procedures as documented in the ICD-9-CM and CPT Official

Guidelines for Coding and Reporting.

Essential Functions

  • Abstracts relevant clinical information from the medical records. Assigns ICD-9-CM and CPT codes

in accordance with coding and reimbursement guidelines.

  • Refers coding issues to Certified Advisory Coding Specialist/Quality Administrator in a timely

manner for determination and guideline development.

  • Maintains a coding accuracy rate of not less than 95% based on internal performance improvement

reviews.

  • Abstracts related demographic and clinical data and enters into computer and ensures that the

be less than 6 records per hour.

  • Ambulatory surgery coding productivity shall not be less than 5-6 records per hour.
