Pay per assessment: $65.57 for telehealth, $100 for in-home (miles reimbursed at 65.5 cents per mile)
Duties:
-Perform health risk assessments which are essentially history and physicals done within the patient's home or on occasion in nursing home or other health facility; on occasion some of these assessments are done through a telehealth visit using simultaneous video and audio communication.
-Clinical documentation of acute, chronic, and long-term healthcare issues through patient encounter in EMR system
-Ensure all appropriate consent forms are signed and dated
-Educate patients and/or patient's family on chronic medical conditions, preventative care, and medication adherence compliance
-Complete referrals to case management, nursing, pharmacy, etc. as needed
-Participate in clinical meetings and compliance trainings which are primarily web based;
-As part of the assessment, the provider must do ADL and Fall Risk evaluation, Depression and Nutritional Health Screening, BMI evaluation, and medication reconciliation
Skills:
-Experience with electronic medical records / electronic health records
-Proficiency with various computer-based applications such as MS Office (Logging into programs, MS Word, MS Excel, MS Outlook, general use of mouse and navigation of different programs, typing) along with utilizing various websites to perform the required duties.
-Strong background/interest in primary care and preventative medicine is a plus
-Effective leadership and interpersonal skills; facilitates problem resolution and maintains a professional demeanor in difficult situations
Other Requirements:
-Must have use of own car with adequate automobile insurance
-Must have home internet connection
-Vaccination requirements: Must be fully vaccinated in this patient-facing role.
-Login requirements: CAQH profile, Medicare/Medicaid profile, PECOS profile
Education:
-Training as a Family NP is required.
-Candidates must be board certified by the ANCC or AANP
-Must have active/unrestricted license in state of position opening
-Must be able to be credentialed by Medicare, Medicaid, and other Private Insurance Companies
