We are looking for Medical Social Workers to work in a hospital based skilled nursing and rehab center located in the heart of San Francisco, CA.Medical Social Worker Responsibilities and Duties: Under general supervision, the Medical Social Worker performs routine medical social work duties; evaluates the social, emotional, and physical needs of clients; interviews clients and their families and significant others; provides supportive counseling or crisis intervention; conducts mandatory reporting activities; advocates on behalf of clients; provides discharge planning, service coordination, psychoeducational, and case management; and performs related duties as required.Education:Master's Degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE);Experience:1 year of Discharge Planning or Case managementLicensure/Certification:LCSW PreferedBenefits:Medical Insurance Paid sick daysMedical Social Worker Pay: $60/hrHours: Monday - Friday 8:30 am - 5:00 pm with some weekend availabilityINDH
