TITLE: Healthcare Service Care Coordinator III
CLIENT: Superior Health Plan
PAY: $24.00-26.00 (per Hour)
BENEFITS: Medical-Dental-Vision (eligible day 1)
HOURS: M-F 40.00-HRs (weekly)
SHIFTS: 8-HR shift 8-5pm CST
TERMS: Contract-To-HIRE (3-Months)
START-DATE: February or Early March 2023

IDEAL CANDIDATE:
3+ year experience in social work (non licensed)
Bachelors in Social Work, Health Services, Behavioral Science

POSITION PURPOSE: _Provide support to medical, behavioral and social support staff to determine needs, eligibility and services for members. Conduct screening and risk assessment interventions per program guidelines._

JOB RESPONSIBILITIES & DUTIES:
Identify special needs members through the completion of health screens and other resources.
Work with community outreach/member advocates to coordinate member care.
Educate providers and community resources on program components and available support services.
Conduct site visits as appropriate for programs and provide support to other special programs.
Assist with modification and management of care plans
