    POSITION:                                    Facility Program Manager  (Exempt), Burlington, NC

    SUPERVISED BY:                        Director of Facility Services

    POSITIONS SUPERVISED:          Occupational Training Specialist      

BA required, QP Status Required

3 yrs professional experience preferred (minimum 2 yrs.

with IDD population, coaching & training)

Knowledge and experience in the field.

   LOCATION:                                    Asheboro, NC

   SALARY RANGE                          $40,000-$48,000 D.O.E        

 

OE is seeking to fill the position of Facility Program Manager in Burlington NC. The person selected for this position is compassionate, reliable, ethical, and kind when providing support to staff and clients. Requires North Carolina Qualified Professional (QP) status.                         

   RESPONSIBILITES & DUTIES

      Supervision of Staff

  • Monitor staffing levels and workload assignments
  • Site and program responsibility for goals and outcomes
  • Responsible for all safety/security guidelines and adherence at facilities
  • Monitor Client/family satisfaction and treatment outcomes.
  •  Ensure the use of person centered planning

Supervision of Employee Services

  • Review all consumer service/grid notes within a timely manner
  • Initiate Authorizations and reauthorizations
  • Meet with Team to create and update ISPs
  • Review all verification/documentation of services for appropriate signatures and documentation per the NC DHHS Records Management and Documentation Manual
  • Ensure assigned direct care staff records are current
  • Ensure assigned service recipient clinical records are current
  • Conduct and document clinical and employee supervisory meetings
  • Conduct annual performance evaluations, direct care staff development plans, disciplinary meetings/documentation with direct care staff
  • Participate in required staff meetings
  • Participate and advocate for the recipient in the recipient’s ISP meetings
  • Manage and supervise staff to ensure development of individualized short term goals, task analysis and interventions for assigned recipients
  • Manage and ensure completion of recipient’s annual skills and preference assessments (as applicable)
  • Ensure client specific trainings are completed for every recipient/staff relationship
  • Ensure that staff enter all documentation accurately in (EHR) Set Works
  • Manage and ensure that the submission of TARS and authorizations are accurate and timely
  • Review weekly billing to ensure hours are used or complete deviation of service forms
  • Ensure completion of quarterly progress reports
  • Ensure staff review and update task analysis per recipient’s goal frequency and as needed
  • Ensure staffing patterns are covered and back up staff is available facility based services
  • Provide OTS  back up
  • Resolve and document recipient/guardian grievances

      General Duties

  • Complete significant event and incident reports within at least 24 hours (recipient and employee/contractor); IRIS reports
  • Provide privacy and confidentiality of all participants’ records.
  • Assure participants are free from abuse, mistreatment, and/or neglect and report any incidents of such to supervisor.
  • Collaborate with VR/Community team and provide support; documentation and a good working relationship with community staff.
  • Treat each participant with dignity and respect.
  • Provide a positive atmosphere which facilitates growth and learning of coping skills.
  • Schedule direct care staff/contractors to provide direct care. Hours must comply with service authorizations and waiver limitations
  • Assist the recipient and families with finding and procuring appropriate resources and services in the community
  • Understanding and carrying out of requirements of Medicaid/State/ billing requirements
  • Maintain Therapeutic working relationship with Service Recipients, Family Members, Team Members
  • Perform daily activities per company’s needs
  • Any other duties as assigned
